Every time I create a bin mesh object in 3dsmax I have to rotate it 90 degrees on Z to get it to match the scene orientation. Is this normal or am I missing something?

Did you check your axis orientation? I assume it's set to RF's default setup. To change it, go to
Preferences > General > Axis setup > ZXY (3dsmax, maya)

That is why I'm finding it odd I always have to rotate the mesh in max.
I made a quick test with a teapot oriented to the front view and as you can see in this screen shot http://www.hamercado.com/realflow/orientation.jpg when I create the BIN mesh object it is oriented from the left side so I always need to rotate to 90 on Z to match my scene orientation.
Am I still missing something?

Well, in this case I think it's a plugin error. I'm on C4D so I can't test it, but I'll ask my colleague Luis to take a look at. He's our 3DS specialist.

Yeah, that's normal. Meshes are always rotated 90º in 3dsmax when importing. It is because of 3dsmax plugin and not a bug when exporting from RealFlow. We are checking this but it will be necessary to rotate the imported mesh for now.
